If you were interested in expanding it maybe add bigger enemy's that take a few shots to kill or something like that.
Id suggest making some particle effects to show the bleeding, overall i really like the idea tho its a fun little game that fits the theme.

May I suggest making the pickups red instead of yellow to represent the blood, and have the enemies a different color.
